Itkovian serves as a warrior-priest within the Grey Swords, a mercenary company defending Capustan against the Pannion Domin. He holds the unique title chosen by his patron god, Fener: to take on the suffering of the world. During the siege, he confronts Anaster, the leader of the cannibalistic forces. Itkovian's presence and ability cause Anaster to flee in terror. The Grey Swords are eventually overwhelmed by the sheer numbers of starving Pannion peasants stirred into a frenzy. With Fener no more due to events occurring shortly before in Deadhouse Gates, the company turns to other gods for patronage. Later, Itkovian is remembered when his memories of the pain of the T'lan Imass are used to seed the warren of Tellann with fertility, preparing it to receive the elder gods Togg and Fanderay as lords of the Beast Hold.

At Capustan, the city burns under the siege of the Pannion Domin. Itkovian stands against Anaster, the leader of the cannibal's forces. He confronts the warlord directly, and Anaster flees in terror from Itkovian's ability to take on suffering. The Grey Swords fight an able defense but are ultimately overwhelmed by the enemy numbers. Later, after the battle, the Mhybe ages rapidly until Silverfox prepares the warren of Tellann. This preparation involves seeding the warren with fertility using Itkovian's memories of the pain of the T'lan Imass who gather to Silverfox's call.
